This helps DMJ

1. write the author bios to accompany the piece,
2. provides us with your mailing addresses to ensure you receive a copy of the journal with your article,
3. provides DMJ with any instructions for included graphs or images you have included.

Please also supply a high-resolution headshot to be included with your bio in the article.
